Appointments/Fees

• Initial Office Visit: $95
• Return Office Calls: $75
• A cash discount of 10% will apply to the above fees if paying by cash/check.
• A seniors (65+) and student discount with ID of 20% will apply to the above fees if paying by cash/check.

24 Hour Notice is required for all cancellations or rescheduling.
A $25 cancellation fee is charged for late cancellations and No-Shows.

I currently do not accept insurance but would be happy to provide you with a superbill so that you can request payment from your insurance company for my services as an out of network provider.  Many insurance plans do offer out of network benefits so contact your plan to inquire about your specific policy. Many patients with out of network benefits prefer to pay the discounted cash/check rate and then submit their own insurance claims for reimbursement.
